 • The presentation is neat & clear. It is formatted well. All care has been taken to deidentify patient's details. 

• Localisation of the abdominal pain & related details were well described.

• History taking has been thoroughly done. All the questions one might have regarding HOPI, past illness, medical/ surgical history, personal history, social & educational history, immunizations have been asked and noted well.

• General Examination was detailed & well documented. 

• The clinical images and investigation reports are extensive and adequate. 

• Diagnostic approach and treatment was also well explained.

CASE 1 

Problems list 

1. Generalized edema (glomerulonephritis)
2. Joint pains (rheumatoid arthritis)
3. Frothing of urine
4. High blood pressure of 150/90 mmHg 
5. Burning sensation in eyes with increased tears 

Treatment Approach: 

1. For Hyponatremia : free water restriction
2. Tab prednisolone p/o 20 mg OD for allergies, inflammatory conditions & autoimmune disorders. 
3. Tab . FEBUXOSTAT P/O 80 mg OD for long term treatment of for due to high uric acid levels.
4. Hemodialysis for improving renal function. 


CASE 2 

Problems list

1. CC involuntary tremors with testing tremors of right hand index and middle finger.
2.  Loss of sensation in lower limb with muscle stiffness
3. Difficulty in speaking
4. Hypertonic major muscle groups
5. High BP of 190/110 mmHg. 

Treatment Approach: 

1. For dopamine: Tab. Syndopa Plus was given
2. Tab Telma 40 mg for high blood pressure

CASE 3 

Problems list 

1. Itchy Lesions over arms, abdomen, thigh, groin 
2. Purple striation over anterior abdominal wall, lower back
3. Weight gain 
4. Pedal Edema 
5. High BP of 160/100 mmHg
6. Low libido

Treatment Approach: 

1. Ointment Amorolfine for decreasing ergosterol levels in the body. 
2. Tab Shalcal 500 OD and tab vit D3 OD for low calcium levels
3. Tab ULTRACET for inflammation

I have had a positive experience this month. There were opportunities to attend rounds in the ICU & see, touch and talk to the patients for ourselves and that was an invaluable learning experience in medicine. While making elogs we could actually go and ask the patient for more details wherever needed which was great. The case discussions in the group are insightful and are a great way to read & comprehend many cases at once in a short span of time, which if done by ourselves would take much longer.
